Rehabilitation RTW Plan

This form supports the Injury Management Procedure, and is included in the Rehabilitation Pack to be provided to the treating medical practitioner.  This is a Return to Work Plan template for workers with more serious or long-term injuries, developed with practitioners and the supervisor to ensure a smooth transition back into the workforce.
