Suitable Duties Capability Statement

This form supports the Injury Management Procedure, and is part of the Rehabilitation Pack provided to the treating medical practitioner. It is to be completed by the practitioner to provide guidance to the organisation on any restrictions placed on the injured worker to allow them to undertake suitable  meaningful duties during their rehabilitation.
