Supplier Risk Assessment Checklist

This form supports the Purchasing and Contract Management Procedure, and is used to risk assess new products from a supplier, or the scope of work for new services and tasks undertaken by a contractor.  Based on the realised risks, controls are identified and addressed with the supplier/contractor before the item is supplied or work commences.
