PPE Issue and Approval Form

This form supports the Personal Protective Equipment Procedure and is used to approve the reissuing/replacement of PPE (without a purchase order) and for the approval of acquiring PPE not previously approved for use either as a trial or an addition to existing PPE.  It is used to assess the validity of the request and ensure purchases are cost effective and beneficial in reducing risk of exposure.
