Permit to Work Procedure

The purpose of this document is to establish a formal process for ensuring a safe work environment and safe system of work is applied for all persons conducting defined work at your facilities. Correct use of the Permit to Work System during planning, risk assessment, isolation of energy sources, and work execution will ensure that the risk to personnel, the environment and assets from hazardous energy sources are reduced So Far As Reasonably Practicable (SFARP) allowing work to progress safely.
The Permit to Work System shall achieve the following outcomes:
- Control access to equipment, systems and areas of the facility
- Control work activities and eliminate or minimise the risk of injury to people
- Mitigate aspects that may adversely affect the environment, equipment and assets
- Comply with relevant Legislation and Codes of Practice
The system ensures appropriate controls are in place prior to undertaking work. The PTW System is used to/for:
- Operational sites (Brownfield sites) and new project sites (Greenfield sites) as applicable
- Detail work to be performed safely and any relevant requirements, risk controls and approvals
- Schedule and manage work effectively to eliminate incompatible tasks being undertaken simultaneously
- Control work activities and manage work related hazards to eliminate or minimise risks to people, the environment, equipment and assets
- Ensure that work is independently reviewed and authorised prior to the commencement of site activities
- Ensure effective communication and notify relevant personnel of all work being carried out
- Verify all necessary controls have been taken prior to the commencement of work and all ongoing risk controls are properly implemented
- Provide an structured method to confirm work is complete and equipment and services are returned to their operational state
The PTW System also provides a trigger for the use of other Subsidiary Permits for specific high-risk work such as hot work, work at heights, confined spaces work, excavation, critical lifting operations, isolation, pressure testing, and removal of grid mesh and hand rails.
