Confined Space Risk Assessment

This form supports the Confined Space Entry Procedure, and is used to record the assessment of all confined spaces, to understand their attributes (configuration, contents, access, isolation points, etc.), tasks, hazards and risks associated with the space, and what controls are required for any activity, including entry, associated with the confined space. A confined space risk assessment is a requirement of AS2865.
